ESIGN DISCLOSURE AND CONSENT

It is required by law to provide you with certain disclosures and information about the products, services or accounts you may receive or access in connection with your relationship with us including but not limited to notices, agreements, and disclosures in connection with your deposit or loan account and other such documents as Country Bank may deliver from time to time (Required Information). With your consent, we can deliver Required Information to you by a) displaying or delivering the Required Information electronically; and b) requesting that you print or download the Required Information and retain it for your records.

This notice contains important information that you are entitled to receive before you consent to electronic delivery of Required Information. Your consent also permits the general use of electronic records and electronic signatures in connection with the Required Information.

After you read this information, if you agree to receive Required Information from us electronically, and if you agree to the general use of electronic records and electronic signatures in connection with our relationship, please click the “I ACCEPT” button below.

Before you consent to receive these documents electronically, we want you to know:

• You have the right or option to receive Required Information electronically or by paper copy delivered by S. mail; both types of Required Information contain the same information.
• If you consent to electronic delivery, you will cease to receive a paper copy by S. mail.
• Your consent to electronic delivery of Required Information will apply to your relationship with Country Bank.

After consenting to receive these documents electronically, you may, upon request, obtain a paper copy of Required Information at no cost to you. If you consent and change your mind later, you may withdraw this consent at any time at no cost to you. To fulfil these requests, you can connect with us in the following ways:

If you would like to print these electronic documents you will need a printer. You will also need the most recent or supported version of one of the following web browsers: Microsoft Edge, FireFox, Google Chrome, or Safari; Email service and an Email address; the most recent or supported version of Adobe Acrobat Reader. Adobe Acrobat Reader may be downloaded at no cost on the Internet from Adobe’s website, but all other software, hardware and systems must be provided at your cost.
• You must provide Country Bank with a current, valid email address. In the event you change your email address, you must notify us by calling our Customer Care Center at 1-800-322-8233.

• If we are repeatedly unable to deliver your Required Information to the specified email address, Country Bank reserves the right to terminate the electronic delivery of documents and deliver documents to you via U.S. mail.
