Individuals will need one primary form of identification. Alternatively, if one of the primary forms of ID cannot be provided, two secondary forms of identification are required. Minors will need two secondary forms of identification, one of which must be a birth certificate. If guardian is not listed on birth certificate, proof of guardianship is required in addition to two forms of identification.
Primary
- Unexpired US government-issued photo ID (e.g., driver’s license, passport, military or firearms, permanent resident card, employment authorization card)
- Unexpired Foreign passport with photograph.
Alternative (2 or more)
- Social Security Number or Taxpayer Identification Number.
- Birth certificate for individuals under 18.
- Other Vital Record or official court document
- Immigration Documents
For US Citizens and Resident Aliens:
- An unexpired Government Issued ID with Photo
- Exceptions may be made on a case-by-case basis
For Non-resident Alien
- An unexpired Government Issued ID with photo plus one alternative bank approved document
- Exceptions may be made on a case-by-case basis
